Ganciclovir sodium is first phosphorylated to monophosphate by the CMV-encoded (UL97 gene) protein kinase homolog and further phosphorylated to diphosphate and triphosphate by cellular kinases. In CMV-infected cells, triphosphate levels were 100-fold higher than in non-infected cells, suggesting that Ganciclovir sodium may be preferentially phosphorylated in infected cells. Ganciclovir sodium, once triphosphate formed, persists in CMV-infected cells for several days. The triphosphates of Ganciclovir sodium are thought to inhibit viral DNA synthesis by: 1. Competitively inhibiting viral DNA polymerase; 2. Co-entry into viral DNA, resulting in termination of viral DNA elongation.
