Researchers found that the main neurogenic and memory enhancing effects from Cerebrolysin were due to its ability to react to human CNTF. So the researchers narrowed their search, and found that an 11-mer peptide, labeled peptide 6 (Ac-VGDGGLFEKKL-NH2), enhanced hippocampus-dependent learning and memory, increased neurogenesis, and neuronal plasticity. They then determined the most active sub-sequence in the peptide called peptide 6C (Ac-DGGL-NH2). However, this peptide sequence showed to have poor blood brain barrier permeability, and was easily broken down by plasma enzymes. To combat this, they added an adamantane moiety to the N-terminus of the peptide sequence. P21 is a cell cycle inhibitor that is involved in essential cellular processes such as cell cycle arrest, apoptosis, and transcriptional regulation, as well as differentiation, senescence, and DNA repair. Because of its role in these processes, a large body of research has studied its role in cancer initiation, progression, and response to therapy.

Currently, the evidence shows p21 to be related to tumor-suppressing activities as well as, paradoxically, tumor-promoting activities. More research is needed to understand p21's role in cancer, which may lead to the development of new therapeutic techniques.

3. P21 vs Cerebrolysin

While p21 is often referred to as a derivative of cerebrolysin, they are not the same compound. Whereas p21 peptide is a synthetic analogue of CNTF, cerebrolysin is a peptide mixture with CNTF being only one of its components. P21 is comprised of only four amino acids from CNTF and is shown to be more efficacious than cerebrolysin, as per animal studies.

In recent years, scientists have abandoned cerebrolysin treatment due to it stimulating the production of antibodies against CNTF and gradually becoming completely ineffective. In fact, it can even make things worse.

At one time, this product was one of the best options one could buy. As a complex blend of different brain chemicals, it was initially an effective treatment. However, it produces high levels of foreign CNTF that causes the production of auto-antibodies, leading to adverse effects.

P21 is proven to confiscate these auto-antibodies and help to diminish them, resulting in enhanced efficacy of native CNTF. Studies show it has not shown to lose efficacy over time, either, which is a very good sign.

All in all, when it comes to the cerebrolysin vs P21 debate, the latter is the clear winner.
